Behavioral Health Clinician (LICSW or LCMHC)
About The Position
DMC Primary Care, a physician-owned, independent practice with offices throughout southern New Hampshire, has been providing comprehensive care for entire families since 1964. We help each patient achieve the best possible health through every stage of life. We do this by providing an accessible, innovative healthcare experience that is built around our patient’s needs. We are seeking a part-time, experienced Licensed Behavioral Health Clinicians (LICSW / LCMHC) to provide behavioral health services to Level 1 referred patients in an integrated, primary care setting. The Behavioral Health Clinicians are considered a part of the primary care clinical team.
Requirements
- Master’s in Social Work from an accredited college/university
- Currently Licensed in NH as an LICSW or LCMHC
- CPR certified
- Minimum 2 years’ mental health counseling experience in health care setting
Nice To Haves
- Previous EHR experience preferred (including ability to document in electronic medical record)
Responsibilities
- Independently assessing individuals with behavioral health needs and/or substance misuse issues
- Developing treatment plans/interventions for individuals with mild to moderate mental health issues
- Interfacing with medication management professionals as needed
- Providing thorough communication to the primary care physician
