Behavioral Health Clinical Case Manager – New Hire Bonus Available Position Summary: The Clinical Case Manager conducts comprehensive in‑person assessments and develops individualized care plans for members in MLTSS and D‑SNP programs. This role coordinates physical, behavioral, and social services across providers and community resources, ensuring members receive appropriate, high‑quality, and cost‑effective care. The Case Manager authorizes services, documents care activities, participates in interdisciplinary meetings, and supports team development. With flexible teleworking and meaningful community engagement, the position offers the opportunity to build trusted relationships, advocate for vulnerable populations, and apply strong clinical judgment while pursuing ongoing professional growth, including NJ Choice Certification. Requirements

  • Licensed Clinical Social Worker (LCSW), or Licensed Social Worker (LSW).
  • 2+ years of Behavioral Health experience/expertise required.
  • Must possess reliable transportation and be willing and able to travel up to 75% of the time.
  • Successful completion of the NJ Choice certification is a contingency of employment. As per NJ Division of Aging Services, Office of Community Choice Options guidance, an exam score of 80% or higher is required, along with successful completion of the State training modules, and field mentoring component.

Responsibilities

  • Conduct comprehensive, face-to-face assessments for members enrolled in MLTSS and D-SNP programs
  • Develop and implement personalized care plans that address physical, behavioral, and psychosocial needs
  • Collaborate closely with members, families, providers, and interdisciplinary teams to coordinate care
  • Evaluate and facilitate services such as: Adult and pediatric medical daycare, Personal care assistance, Nursing facility support, Community-based and long-term services
  • Advocate for members to ensure access to appropriate and high-quality care
  • Authorize services and ensure efficient, cost-effective resource utilization
  • Document care activities accurately in the electronic health record
  • Participate in interdisciplinary meetings and help drive positive outcomes
  • Support and mentor new team members as you grow in your role
