About The Position

Mercyhealth is seeking a full-time Behavioral Health Case Manager to consult and coordinate patient care with physicians, psychiatrists, RNs, social workers/therapists, and other agencies. This role involves assessing treatment planning and progress, preparing treatment plans and goals, maintaining patient data in the EMR, providing psychoeducational groups, and arranging for disposition and follow-up recommendations.

Requirements

• Master's Degree in a mental health or behavioral health field
• Experience or field study experience in mental health site.
• Exceptional communication skills

Nice To Haves

• Working toward licensure is preferred.
• TACT certified within 90 days after hire.
• Approved CPR/BLS preferred

Responsibilities

• Consults and coordinates care of patient with the physician/psychiatrist, RN, social worker/therapist and other agencies.
• Assesses treatment planning and progress.
• Prepares treatment plan and goals for patients.
• Maintains pertinent patient data in the EMR in accordance with state and federal laws and hospital policy.
• Provides psychoeducational groups.
• Arranges for disposition and follow up recommendations.
