About The Position

Ocracoke Health Center, Inc. is seeking a Behavioral Health Case Manager to join their Integrated Care Team. This role involves working closely with primary medical providers and a Behavioral Health Counselor to manage referrals, connect patients to care, and address social determinants of health. The position is part-time, approximately 20 hours per week, with a hybrid work arrangement that includes scheduled onsite days in Engelhard and Manteo. Flexibility exists in establishing the onsite schedule.

Requirements

  • Bachelor's degree in social work or a related human services field required.
  • Candidates with an associate degree or actively pursuing a bachelor's degree and expected to graduate within one year may be considered.
  • One or more years of experience in case management preferred, particularly in a community-based or healthcare setting.
  • Strong verbal and written communication skills.
  • Experience with and/or ability to learn and use electronic health record systems (Athena experience is a plus).
  • Commitment to maintaining patient confidentiality and professionalism.
  • Knowledge of community resources and human service systems across the lifespan.
  • Ability to work collaboratively with patients, healthcare providers, community partners, and other members of the care team.

Responsibilities

  • Receive and manage referrals from primary care providers and behavioral health counselors.
  • Coordinate care and provide case management services, including assessment, care planning, follow-up, advocacy, and resource coordination.
  • Connect patients with specialty care, behavioral health services, and community resources that support their health and well-being.
  • Assist patients in addressing social determinants of health, including barriers related to housing, transportation, food access, financial stability, and other community support needs.
  • Collaborate closely with medical providers, behavioral health counselors, and other members of the care team to support patient goals and improve outcomes.
  • Provide administrative and care coordination support to the Behavioral Health Counselor and Integrated Care Team.
  • Maintain accurate, timely, and compliant documentation in the electronic health record (Athena).
  • Participate in team meetings, case reviews, and care planning activities.
  • Deliver patient-centered, culturally responsive services that respect the unique needs and circumstances of each individual.
