Summary Coordinates and supports mental health care to patients enrolled in Collaborative Care Management (CoCM) by providing evidence based, short term interventions, case management needs, and acting as a core member of the CoCM team (which includes the primary care provider and a psychiatric consultant).

Requirements

  • Bachelor’s Degree In social work, psychology, counseling, nursing or other related mental health field.
  • 2 years In mental health/behavioral health experience or related setting.
  • Formal education or specialized training in behavioral health, which could include a range of disciplines including social work, nursing and psychology.

  • Demonstrated knowledge and experience in a behavioral health field.
  • Ability to work effectively with people from a broad range of backgrounds.
  • Must possess a high degree of emotional intelligence, and empathy towards others.
  • Skill in using logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions, or approaches to problems.
  • Knowledge of evidence-based interventions such as Motivational Interviewing, Problem Solving Techniques, and/or Behavioral Activation.

Responsibilities

  • Supports and closely coordinates mental health care with the patient’s primary care provider, with recommendations from a psychiatric consultant.
  • Screens and assesses patients for common mental health needs, including regularly administering assessments to monitor progress and treatment intervention efficacy (treatment to target).
  • Monitor patients (via telephone, virtual outreaches, and in-person as needed) for changes in clinical symptoms, medication treatment adherence, side effects and other concerns that can then be communicated to the Primary Care Provider who is prescribing the medications.
  • Provide brief evidence-based interventions such as Behavioral Activation, Problem Solving Techniques, Motivational Interviewing skills and other skills appropriate for primary care settings.
  • Provide and coordinate referrals (outside of Primary Care) to treatment or psychosocial needs as clinically indicated.
  • Participate in regularly scheduled (weekly) systematic case reviews with the psychiatric consultant.
  • Communicate recommendations to the Primary Care Provider as needed for those not improving as expected.
  • Track/document patient follow-up, clinical outcomes, and progress/treatment recommendations or issues in an EPIC based registry to enhance communication with Primary Care Providers and other treatment providers.
  • Complete relapse prevention plans with patients who are in remission for their mental health needs.
