About The Position

This position provides Case Management services to a caseload of approximately 25 Participants, applying the Housing First approach. The role involves active participation in The Louisiana Balance of State Continuum of Care (LA BOSCOC), submitting routine reports and outcome data utilizing the HMIS system, and conducting necessary assessments. The Case Manager will complete mandated legal documentation and Individualized Service Plans (ISP) according to funding source and policy timelines. Responsibilities include planning, scheduling, and coordinating meetings with Participants and other providers, as well as researching and linking participants to appropriate medical, psychiatric, education, community, and other social services/resources. Collaboration with other providers, leasing managers, and landlords is essential to ensure appropriate service provision. The role requires regular meetings with a supervisor and active participation in staff meetings and required training.

Requirements

  • Bachelor’s Degree in Social Service or Human Services related field.
  • Must have their own reliable transportation.
  • Willing to travel during the day to outlying parishes as required.
  • Able to multitask and meet deadlines.
  • Excellent written and verbal communication skills.
  • Proficiency in the use of Microsoft Office Software (i.e. Word, Excel) and computer literate.
  • Must be a team-player.
  • Assess needs and consider course of action to best meet those needs.
  • Monitor participant progress and respond to unmet needs; able to monitor provider service provision and billing practices and to ensure accuracy and efficiency.
  • Establish effective objectives, priorities, and implement plans; anticipate obstacles and consider alternatives.
  • Adjust in casework to coordinate efforts with other agencies to ensure consistency and adequate follow-up.
  • Take initiative rather than passively accepting; acts and attempts to influence events to achieve goals.
  • Willingly commits extra effort and time to get a job done; identifies with organization goals; exhibits excellent time management abilities.
  • Ability to exhibit appropriate use of grammar and spelling in written communication, exhibit appropriate use of grammar in oral communication; ensure oral and written communication is professional.
  • Able to establish rapport with provider agencies and staff, Easterseals administrative and other program staff.
  • Work cooperatively with other Easterseals Support Coordinators/Supervisors.
  • Ability to identify with project and Easterseals objectives; adequately, constructively, and directly communicate these objectives.

Responsibilities

  • Participate as an active member of The Louisiana Balance of State Continuum of Care (LA BOSCOC).
  • Submit routine reports and outcome data, per contractual requirements, and utilize the HMIS system.
  • Conduct necessary assessments, complete mandated legal documentation, and complete Individualized Service Plans (ISP) according to funding source and policy timelines.
  • Plan, schedule, and coordinate meetings with Participants and other providers.
  • Research, access, and link to appropriate medical, psychiatric, education, community and other social services/resources, as indicated by assessments and individual participant preferences.
  • Communicate and work collaboratively with other providers, leasing managers and/or landlords to ensure appropriate service provision.
  • Meet with the supervisor for regularly scheduled meetings and as assigned by the supervisor.
  • Attend and actively participate in regularly scheduled staff meetings and as assigned by the supervisor.
  • Attend and actively participate in required training, as assigned.
  • Work with the participant, his/her identified support systems to set long-range and short-range goals based on identified needs; ensuring professional recommendations including but not limited to physician, psychiatrist, counselor/therapist(s) and other service provider(s) are incorporated into goals and outcomes.
  • Ensure delivery of services meets the established intensity and frequency as clinically indicated.
  • Ensure documentation and required reports are completed within timelines as required by funding source, policy, and supervisor.
  • Make referrals to appropriate medical, psychiatric, education, community resources and other social services agencies and/or providers in a timely manner to address identified needs.
  • Document and follow-up in a timely manner to ensure services are in place and appropriate.
  • Establish and maintain ongoing rapport with participants and his/her identified support systems to assist participants in achieving his/her identified goals.
  • Establish and maintain ongoing rapport with referral sources, other community agencies and providers to coordinate services for participants.
  • Respond to referrals and requests for services within timelines as required by funding source, policy, and supervisor.
  • Empower and encourage participants and his/her identified support systems to be active partners in services.
  • Ensure all participants, and their significant others, are treated with respect and dignity; and to ensure their rights are preserved.
  • Maintain a minimum 40-hour work week.
  • Actively participate in case reviews, team meetings, staff meetings and other training as required by funding source, policy and supervisor.
  • Ensure records, plans, legal forms, and other documentation are kept accurate and up to date.
  • Document all services provided and appropriately signed and dated and in the appropriate location in the chart based on funding source, policy, and supervisor. Documentation reflects current services, interventions, participant participation, progress towards goals and/or any challenges or barriers to achieving goals.
  • Participate in training activities and programs as required by funding source, policy, and supervisor.
  • Participate in trainings for certifications, credentials or specialties as determined appropriate by Easterseals Louisiana, funding source or supervisor.
