About The Position

The Grant Behavioral Health Care Navigator is part of an interdisciplinary team and responsible for completing enhanced screening, connecting families to onsite resources at SCHC and offsite referrals, and maintaining contact with families to ensure barriers to receiving services are addressed. The behavioral care navigator role is intended for a person with lived experience. Lived experience is broadly defined to include individuals that are trained as a Community Health Worker, Family Peer or Certified Peer Specialist. Requirements

  • Completes and maintains all competencies and trainings as required for role.

  • Multitasking
