About The Position

Every day, patients walk through our doors carrying more than physical health concerns. Many are struggling with anxiety, depression, substance use disorders, grief, stress, or other challenges that affect their overall well-being. Too often, behavioral health needs go untreated because access to care is limited. At High Rock Internal Medicine, we believe mental health is healthcare. That's why we're looking for a compassionate Behavioral Health Care Manager to help bridge the gap between physical and behavioral health services. In this role, you'll become a trusted partner for patients who need support navigating life's challenges while improving their health outcomes. Working within the Collaborative Care Model (CoCM), you'll have the opportunity to make a meaningful difference in patients' lives by providing evidence-based interventions, coordinating care with medical providers and psychiatric consultants, and helping patients achieve healthier, more fulfilling lives. Requirements

  • Master's-level licensure in a behavioral health field (LCMHC, LCSW, LMFT), or associate-level licensure (LCMHC-A, LCSW-A, LMFT-A)
  • Experience with mental health screening, assessment, and treatment planning
  • Strong communication and relationship-building skills
  • Comfort working with patients both in person and by phone
  • Familiarity with evidence-based behavioral health interventions and psychopharmacology
  • Strong organizational and documentation skills
  • A compassionate, approachable, and patient-centered mindset

Responsibilities

  • Help patients access behavioral healthcare within a trusted primary care setting
  • Reduce barriers to treatment for individuals who may not otherwise seek help
  • Support patients managing chronic medical conditions impacted by behavioral health concerns
  • Work collaboratively with physicians, advanced practice providers, and psychiatric specialists
