About The Position

The Behavioral Health Care Manager, Telephonic Nurse, in a telephonic environment, assesses and evaluates members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Care Manager, Telephonic Nurse work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action. Humana is seeking a Behavioral Health Care Manager to telephonically assess, evaluate, and support members who are challenged by serious mental illness and/or substance use. Our goal is to provide support for both mind and body to allow each member to achieve and/or maintain their optimal health. The Behavioral Health Care Manager, Telephonic Nurse employs a variety of strategies, approaches, and techniques to manage a member's physical, environmental, and psycho-social health issues. Identifies and resolves barriers that hinder effective care. Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluations. Understands department, segment, and organizational strategy and operating objectives, including their linkages to related areas. Makes decisions regarding own work methods, occasionally in ambiguous situations, and requires minimal direction and receives guidance where needed. Follows established guidelines/procedures.

Requirements

  • Minimum 2 years in an integrated care setting managing members with both Behavioral Health needs and/or Substance Use such as crisis experience (includes ER), community mental health center, behavioral health inpatient, assertive community treatment, etc.
  • Active Registered Nurse (RN) license and possess the ability to be licensed as a registered nurse in multiple states without restrictions
  • Proficiency with Microsoft Office Programs, e.g. Word, Excel & PowerPoint, shared systems, troubleshoot and resolve general technical issues
  • Self-provided internet service with at minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps; wireless, wired cable or DSL connection is suggested.
  • Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.
