About The Position

A behavioral health care coordinator is a specialized team member who helps rural patients navigate mental health and substance use disorder treatment along with bridging the gap between mental health facilities and primary care providers.

Requirements

• Bachelor’s degree in social work, psychology, counseling, public health, or a related field. (Some positions allow relevant case management experience to substitute for a degree).
• 2 to 3+ years of experience working with high-need populations, health navigation, or behavioral health/social services.
• Strong verbal and written communication skills
• Cultural competency for diverse rural settings
• Proficiency with electronic health records (EHR) or care coordination software.
• Valid driver’s license
• Auto insurance
• Background clearance

Responsibilities

• Develop personalized care plans, coordinate monthly telephonic/telehealth check-ins, and navigate patients through chronic care management (CCM).
• Help patients find local clinics, mobile outreach units, or regional behavioral health centers.
• Connect rural primary care providers with regional psychiatric consultants, emergency services, and community-based support teams.
• Set up and guide patients through virtual psychiatric consultations and therapy sessions.
• Demonstrate a respectful, open, and unbiased approach toward diverse rural clients, families, and high-need groups.
• Maintain secure, data-driven patient tracking aligned with state and federal compliance metrics.
• Create and track personalized mental health and wellness plans alongside primary care doctors.
• Reach out to patients after an emergency room visit or crisis intervention to prevent readmission.
