Help individuals build healthier lives by connecting behavioral health, primary care, and community support in a collaborative, person-centered role. Schedule: Mon-Fri, 8am - 5pm The Behavioral Health Assistant is an integral member of our interdisciplinary care team, supporting whole-person health for individuals enrolled in our Primary Care Health Home program. Working alongside behavioral health and medical providers, you'll help individuals navigate services, strengthen self-management skills, overcome barriers to care, and stay engaged in their health and wellness goals. This role combines behavioral health support, care coordination, health education, and community resource navigation to improve health outcomes through compassionate, trauma-informed, and person-centered care. While this position does not provide clinical diagnosis or therapy, it plays a meaningful role in helping individuals access the right services at the right time.
