This role involves completing preadmission screening and referral services for behavioral health patients, including psychiatric and chemically dependent patients in various locations and means including virtual, Behavioral Health Facilities, outpatient clinics locations, facility units, outpatient surgical facilities and emergency departments. The assessor will obtain detailed history, document using guided data collection forms, formulate provisional DSM diagnoses, establish patient rapport and trust, obtain appropriate consents, and educate patients/families regarding the process. This position requires knowledge of data elements for assessing all age groups and patient types (excluding neonates) and utilizes assessment tools including video and phone. The role also involves managing time effectively, understanding and complying with various regulations (TDMHMR, TDH, CMS, EMTALA, Texas Behavioral Health Marketing Act), educating others on these regulations, and understanding legal statutes for voluntary and involuntary patients. The Behavioral Health Assessor will recommend levels of care and discharge plans, possess knowledge of community resources, and support continuity of care. Collaboration with facility staff, physicians, and the care team is essential, along with completing other assigned work by leadership, including collaborative workgroups and strategic initiatives.
