As a Behavioral Health Advocate, you will be responsible for case management of mental health and substance use disorder services for those discharging from the hospital, high utilizers of services, and others identified in needing case management support via telephonic contact. You'll have a direct impact on the lives of our members as you recommend and manage the appropriate level of care throughout the entire treatment plan. You’ll enjoy the flexibility to work remotely from anywhere in the U.S. as you take on some tough challenges. The schedule is Monday - Friday, 8am - 5pm CST and includes working one weekday holiday per year.

Requirements

  • Licensed Master's degree in Psychology, Social Work, Counseling, or Marriage and Family Counseling; Licensed Ph.D.; or RN with 2+ years of experience in behavioral health
  • Active, unrestricted, independent clinical licensure in state of residence (e.g., LCSW, LICSW, LMFT, LMHC LP, LPC, LPCC, RN, etc.)
  • 2+ years of experience in a related mental health setting
  • Proficient in MS Office Suite, including Word, Outlook, Excel, and Teams
  • Access to install secure, high-speed internet and a dedicated, distraction free workspace at home
  • Able to work Monday - Friday, 8am - 5pm CST
  • Available to work one weekday holiday per calendar year

Responsibilities

  • Assisting consumers by using clinical skills such as motivational interviewing, strengths perspective, risk assessments, coaching and other clinical modalities
  • Refer members to appropriate psychiatric and community referrals and assist with identifying and removing potential barriers
  • Discharge planning or management of transitions between care settings to ensure the appropriate services/resources are in place for quality outcomes
  • Comply with all policies, procedures, and documentation standards in appropriate systems, tracking mechanisms and databases
  • Assistance to consumers by screening, answering questions, providing program information and education about network and community resources
  • Management of phone queue including a crisis line
  • Participate in standing meetings to review performance, identify areas of opportunity, and oversee course for improvements
  • Collaborate with providers to determine mental health concerns, barriers, and progress; as well as address basic needs and gaps in care
  • Contribute to treatment plan discussions; routinely attend clinical rounds and other meetings with medical partner or external entities, when applicable
