About The Position

The Bed Readiness RN Referral Coordinator is responsible for managing and coordinating patient referrals across post-acute settings to ensure timely, appropriate placement aligned with clinical needs and system capacity. This role serves as a central liaison between hospitals, care teams, and external partners (SNFs, AFHs, ALFs, respite, etc.) to optimize patient flow and reduce length of stay. The primary purpose is to support safe, efficient transitions of care while maximizing access, utilization, and continuity across the care continuum.

Requirements

  • Bachelor's degree in Nursing, or obtained within five (5) years from the initial date of hire; RN with 15+ years of service or internal candidates hired before January 1, 2021 may be considered in satisfaction of this requirement in lieu of an education agreement
  • Current Registered Nurse license in Washington State or Multistate License endorsement (MLS)
  • Current BLS Certification required
  • ACLS or NALS or PALS or TNCC or NRP certification, as required by facility or department

Nice To Haves

  • Advanced degree in Nursing (MSN, PhD, DNP, ARNP) preferred
  • Certification in specialty area preferred
  • One (1) year of experience in acute care setting preferred

Responsibilities

  • Coordinate and manage all incoming and outgoing referrals for post-acute placement, ensuring accuracy, completeness, and timely processing.
  • Assess patient clinical needs and collaborate with interdisciplinary teams as needed to identify the most appropriate level of care and placement options.
  • Serve as the primary liaison between hospital care teams, internal programs, and external partners to facilitate efficient communication and placement decisions.
  • Monitor bed availability, capacity, and placement timelines across the network to optimize throughput and reduce avoidable delays in discharge.
  • Track and analyze referral trends, turnaround times, and placement barriers to identify opportunities for process improvement and system efficiency.
  • Ensure compliance with regulatory requirements, payer guidelines, and organizational policies related to referrals and transitions of care.
  • Support escalation and problem resolution for complex or delayed placements, partnering with leadership and external vendors to remove barriers and ensure patient-centered outcomes.
