This role is responsible for determining the most appropriate level of care and ensuring a smooth admission experience for patients by coordinating clinical needs with operational processes and insurance benefits. Chart review is used to assess for appropriate level of care. The Bed Planner assigns patients to the appropriate units based on clinical assessment while balancing the resources of the organization, coordinating with the unit charge nurse for bed placement and collaborating with the nursing operations leader for throughput issues. This position serves as an access point resource for physicians, staff, and patients, responding to and assigning all bed requests and transfer orders in the electronic medical record. The Bed Planner documents unusual occurrences and patient relations issues, making appropriate referrals to risk management, infection control and quality departments. They utilize appropriate downtime procedures for bed placement when applicable and assist with data collection and reporting for throughput and bed placement. Chart audits are performed as requested by manager/director. The Bed Planner collaborates with the admitting physician on the most appropriate site of care for services to be rendered and any alternate levels of care available to meet the patients’ needs. They coordinate appropriate bed placement for patients with Patient Care Services, collaborating on location, staffing, etc. Utilizing all available tools and resources to gain a better and complete picture of the clinical assessment of the patient, they notify physicians and staff of concerns/variations. Communication is conducted in a non-threatening way that conveys caring and respect for patients, families and co-workers. A complete and accurate hand-off is communicated to the on-coming shift, minimizing miscommunication errors. Admission forms and paperwork are completed completely and thoroughly. Assistance is provided for easy access to incoming patients by arranging transportation when necessary, coordinating services, and notifying points of entry of the patient's impending arrival. Admission is coordinated with registration staff to obtain accurate patient demographics as indicated. Appropriate cases are referred to patient financial services, risk, peer review, infection control and patient relations. Patient activity and flow or movement between levels of care is coordinated. Potential throughput barriers are identified and acted upon proactively to inform and alert upper management of anticipated delays. The Bed Planner confers with MD to discuss clinical and appropriate level of care when needed and participates in process improvement activities, identifying opportunities. An interest in personal growth and development beyond the routine expected educational sessions is demonstrated. New and current co-workers are embraced, contributing to their growth and development.
