Theoria Medical is seeking an Advanced Practice Provider (APP) or Physician Assistant (PA) to serve local skilled nursing facilities in Findlay, OH. Requirements

Must hold an active U.S. license
Must be board certified by ANCC, AANPCB, PNCB, or AACN (for Nurse Practitioners), or by NCCPA (for Physician Assistants)
Qualified specialties include but not limited to Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.

Responsibilities

Provide high-quality, patient-centered care to residents in skilled nursing facilities
Conduct daily rounding, prescribe medications/treatment plans, and address acute/emergent issues
Communicate with facility nursing staff via phone or secure messaging as needed
Maintain reliable on-site attendance for scheduled shifts
Ensure compliance with clinical standards, facility policies, and regulatory requirements
