About The Position

Our network includes multispecialty physician services covering skilled nursing facilities nationwide. We are seeking a Medical Director and Attending Physician to serve local skilled nursing facilities within a mission-driven care community. We emphasize building relationships, consistency, and presence, allowing physicians to care for patients over time, build trust, strengthen continuity, and make a lasting difference for vulnerable populations. Requirements

  • Physicians (MD/DO) must hold an active U.S. medical license.
  • Must be board-certified or board-eligible in an appropriate specialty (Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine are examples).
  • New graduates who are board-eligible and actively pursuing licensing are encouraged to apply.
  • Must be able to perform the essential functions of this position satisfactorily, with or without a reasonable accommodation.

Responsibilities

  • Serve as the attending physician for patients in each assigned SNF.
  • Collaborate with NPs/PAs to manage daily patient care.
  • Provide high-quality, patient-centered primary care, including diagnosing and treating acute and chronic conditions.
  • Participate in Theoria’s Chronic Care Management, Transitional Care, Remote Patient Monitoring, and other quality initiatives.
