About The Position

Anoka County Community Social Services and Behavioral Health is committed to meeting people’s needs through quality services. We are seeking Associate Social Workers in our Disability and Aging Services (DAS) service area. The Associate Social Worker plays a key role in supporting Home and Community-Based Services (waiver programs), Community First Services and Supports (CFSS), Rule 185 Case Management, and the Family Support Grant. This position works closely with DAS Leadership, Coordinators, Case Managers, and other staff to help operationalize these programs and ensure smooth service delivery. Associate Social Workers provide administrative support as well as engage directly with individuals seeking services to gather information and follow eligibility for access to services. Success in this role requires strong attention to detail and communication skills. Requirements

  • Bachelor’s degree in a closely related field. In lieu of the required education, an equivalent combination of education and experience may be substituted on a year-for-year basis.
  • Valid driver’s license and vehicle available for business use.

Nice To Haves

  • Bachelor’s degree in Psychology or Social Work.
  • One year (2,080 annual hours) or more of job-related experience in Human Services, Social Services, Public Assistance, or closely related field.
  • Experience working with Minnesota Medicaid Management Information System (MMIS), Medical Assistance (MA), Social Services Information System (SSIS), Home and Community Based Services, Caseworks electronic filing system, and Microsoft Office Suite.
  • Experience working with vulnerable adults and/or individuals that are affected by issues including neglect, domestic violence, disability, mental health, homelessness and financial insecurities.
  • Strong verbal and written communication skills.
  • Skilled in problem-solving, critical thinking and effective time management.

Responsibilities

  • Assist clients in accessing the Family Support Grant.
  • Assist in processing CFSS Service Delivery Plans.
  • Provide direct and administrative support for client transportation services.
  • Meet with clients in office-based or community-based settings.
  • Assist clients in accessing services by entering screening documents and services agreements.
  • Complete and maintain case notes and records.
  • Communicate effectively with clients, providers, agency staff, and the public.
  • Collaborate with interdisciplinary teams including, but not limited to Leadership, Coordinators, Case Managers and other support staff.
  • Organize, prioritize, and manage workflows.
