About The Position

The Associate Medical Director (AMD) is an entry-level leadership role that will be trained in FQHC compliance and operations, oversight of a community health center, clinical quality and excellence, fiscal planning, budgeting, staff management and supervision, clinical leadership, visioning for clinical care in an integrated setting, and value-based care. The AMD is primarily responsible for the provision of productive and quality clinical practice at the Integrated Community Clinic, supporting and enhancing the practice, including creating efficiencies and innovative healthcare practices. This is accomplished through the monitoring of clinical skills of providers, providing clinical and practice management support, coaching, and mentoring to clinical providers, participation in the performance improvement process, assisting in the development of policies and procedures used to ensure quality patient care and the orientation of new providers, and providing support for strategy for the clinic regarding integration of care across clinic services lines as well as other services in the organization, and move towards value-based care. Required to take call on a rotating basis. May be required to work nights and weekends.

Requirements

  • M.D. or D.O. degree from accredited United States medical school.
  • Successful completion of Internship and Residency from accredited internship/residency program.
  • 3 plus years experience as a primary care provider.
  • Experience in leadership roles preferably in clinical settings such as leading improvement projects, leading committee’s or as a Chief Resident.
  • 1 years experience in supervisory and/or management position coaching and supporting clinical staff development preferred.
  • Board Certified in family medicine or internal medicine.
  • Valid and active California medical license, without any limitations on license to practice in designated specialty.
  • Eligible to participate in the CMS Medicaid/Medicare reimbursement programs.
  • Valid and active DEA license.

Nice To Haves

  • Experience in associate director role.
  • Experience treating with patients with addiction. If no experience, willingness to be trained in addiction medicine as part of their primary care practice.
  • Experience working with patients that have a history with the criminal justice system.
  • Experience with integrating safe management of pain syndromes within primary care.
  • Experience with and knowledge of third-party payer requirements, California practice guidelines and malpractice laws, Title 22, Bureau of Primary Care, and state funding sources preferred.
  • Experience working in FQHC.
  • Bilingual in Spanish.

Responsibilities

  • Contributes to and implements policies pertaining to the HR360 medical care.
  • Provides medical supervision and oversight of quality of care and practice management among providers and ancillary staff in clinical areas.
  • In conjunction with CHO matures services, based on patient population needs.
  • Strategize how best to allocate resources to support expanded services as appropriate.
  • Ensure that in the absence of a primary care provider that a mechanism is established so that a physician is available to review lab and diagnostic reports, prescriptions and follow up on medical care.
  • Fosters integration of services at the ICC clinic including primary care, dental, behavioral health, and addiction treatment.
  • Fosters integration of services between clinic and other HR360 behavioral health programs.
  • Acts as champion for QI efforts and supports providers in meeting QI goals.
  • Member of the QI committee and contributes to setting of QI goals, supporting QI efforts in the clinic.
  • Oversees peer review including ensuring quarterly reviews, monitoring and evaluation, remediation as necessary.
  • Provides supervision of medical providers.
  • Provides leadership for and champions team-based, integrated care model.
  • Reviews employee performance, provides feedback and makes recommendations for individual performance improvement and development for medical providers and other staff.
  • Orients new providers/supervisees, monitors performance during initial employment and thereafter through chart audits, utilization review and other processes.

  • Provides coaching and support to providers/supervisees in clinical practice and practice management.
  • Provides comprehensive care, within the scope of credentialed privileges and standardized procedures, to HR360 patients within a team-based, integrated delivery model.
  • Documents care promptly and following charting protocols and applicable CA state licensing requirements.
  • Completes patient sessions in a timely manner.
  • Ensures appropriate follow-ups for all aspects of ordered care (e.g., specialty referrals, laboratory results, radiology reports, etc.).
  • Responds to patient requests for information and assistance (e.g., form completion, prescription refills, etc.) in a timely manner.
  • Works collaboratively with mental health staff to facilitate integration between primary care and mental health in order to improve patient outcomes.
  • Provides on-call after-hours availability on a rotating basis.
  • Obtains and maintains licensure, DEA registration and BCLS/CPR certification.
  • Attends continuing professional education programs.
  • Works evening and weekends as required.
  • Provides patient care 40% of the time or as agreed upon and approved by the Deputy Medical Officer or Chief Healthcare Officer.
  • Takes call on a rotating basis.
  • Participates in trainings as assigned regarding: FQHC compliance and operations, Quality Improvement, supervision, staff management, etc.
  • Demonstrates leadership growth through yearly goals set in conjunction with the Chief Healthcare Officer.
  • Demonstrates clinical leadership and support for providers as assessed through provider feedback, etc.

  • Participates in ongoing internal and/or external leadership development opportunities.
  • Attends and participates in weekly Multidisciplinary Leadership Team Meetings, QI committee meetings, staff meetings, and other meetings as required.
  • Preceptor for APP residents, medical students, residents, fellows, etc as appropriate and necessary.
  • Provides clinical leadership to high quality care and value based care.
  • Supports and actively participates in clinical performance improvement initiatives.
  • And other duties as assigned.
