Requirements

  • Bachelor's of Science in Nursing (BSN) required
  • New York State RN License required
  • Minimum 7-10 years of direct clinical experience in Case Management, Utilization Management (UM), and Discharge Planning required

Nice To Haves

  • Master’s degree preferred
  • Prior leadership experience strongly preferred
  • Certified Case Manager (CCM) or Accredited Case Manager (ACM) certifications preferred

Responsibilities

  • Provide strategic and operational leadership for the Case Management department, including oversight of discharge planning, utilization management, and care progression activities.
  • Lead, coach, and develop Case Managers and ancillary staff through performance management, competency assessment, mentoring, and ongoing professional development.
  • Foster collaboration across Case Management functions and interdisciplinary teams to optimize patient outcomes, denial prevention, length of stay, and transitions of care.
  • Serve as an expert resource for complex case management, utilization review, and discharge planning issues, providing consultation and guidance to staff and clinical partners.
  • Partner with the Director to establish operational priorities, oversee staffing and workflow, monitor resource utilization, and support departmental budget planning.
  • Oversee the effective use of Epic, CarePort Transitions, InterQual®, and other clinical systems, ensuring accurate documentation and collaborating with technology partners to improve workflows.
  • Ensure compliance with organizational policies, regulatory and accreditation requirements, CMS Conditions of Participation, and Case Management standards of practice.
