Social Work Services at the Mount Sinai Hospital is a comprehensive department within a 1,134-bed facility that offers extensive outpatient and specialty care. It is the largest hospital in the Mount Sinai Health Care System. The department provides a wide range of social work services and programming to meet the diverse needs of the populations served, emphasizing the physical, emotional, and social needs of each patient and their care partners. Social Workers collaborate within interprofessional teams, serving patients and the community at both micro and macro levels, including direct care and prevention. Founded in 1907, the Department of Social Work Services is one of the oldest hospital social work departments in the nation, employing over 450 licensed social workers across more than 65 program/service areas. The department offers a pathway to LCSW licensure and access to continuing education and professional development opportunities. Mount Sinai Health Partners (MSHP) is the management services organization dedicated to enhancing the Health System's implementation of population health initiatives. These initiatives aim to improve the patient experience and health status, enhance the health of patient populations, and reduce utilization and healthcare costs, with a particular focus on preventive care. MSHP provides the infrastructure for these initiatives, including care coordination, workflow optimization, physician engagement, and quality reporting. The Department of Social Work Services plays a leadership role in MSHP Care Management, which delivers in-person and telephonic care management services to a large, diverse group of individuals with various insurances enrolled in value-based contracts. MSHP Care Management employs a patient-centered approach focused on improving care coordination and service integration to manage future healthcare costs and improve health outcomes for this population. MSHP Care Management is seeking an Associate Director to oversee the quality, effectiveness, and efficiency of all program initiatives. These initiatives involve targeted outreach and engagement of high-risk members and the provision of psychosocial interventions to increase connectivity with appropriate providers and services, thereby reducing avoidable inpatient and emergency department utilization. Reporting to the Vice-President of Care Management, the Associate Director will supervise three direct reports (supervisors) and provide clinical direction for all Social Workers and Care Coordinators (30 FTEs). This role also ensures seamless collaboration with the nursing counterpart responsible for Nurse Clinical Coordinators on the Care Management team. The Associate Director will be a key member of the senior leadership team within MSHP Care Management and will serve as a primary point of contact within Mount Sinai Health System, its affiliated practices, and payer partners. This is a hybrid position (remote and onsite work) and requires participation in the leadership coverage team.
