About The Position

The Mount Sinai Health Partners (MSHP) Care Management is seeking an Associate Director to oversee the quality, effectiveness, and efficiency of all program initiatives. These initiatives focus on the targeted outreach and engagement of high-risk members and the provision of psychosocial interventions to connect them with appropriate providers and services, thereby reducing avoidable inpatient and emergency department utilization. Reporting to the Vice-President of Care Management, the Associate Director will manage three direct reports (supervisors) and provide clinical direction for approximately 30 FTEs, including Social Workers and Care Coordinators. This role also ensures collaboration with the nursing counterpart responsible for Nurse Clinical Coordinators. The Associate Director will be a key member of the MSHP Care Management senior leadership team and a primary contact within the Mount Sinai Health System, its affiliated practices, and payer partners. This is a hybrid position requiring both remote and onsite work, and participation in the leadership coverage team.

Requirements

  • NYS LCSW
  • At least 7-10 years of post-MSW experience
  • Experience supervising social work staff within a health care setting
  • Demonstrated leadership experience in innovative programs with complex operational and regulatory requirements
  • Exceptional collaborative, organizational, and communication skills
  • Ability to effectively liaise and work with a variety of internal and external stakeholders
  • Ability to review and analyze complex data relating to care management activities and initiatives
  • Proficiency in data analysis/management and related IT systems
  • Excellent written and verbal communication skills, with the ability to summarize analyses and synthesize information into insights
  • Minimum of one year in current position

Responsibilities

  • Coordinate oversight of clinical and administrative functions of the care management program under the direction of the Vice-President of Care Management for Population Health.
  • Be accountable for the performance of three supervisors and their teams of social workers and care coordinators.
  • Help develop strategy to support program goals and objectives, ensuring alignment with policy and regulatory compliance.
  • Provide decision support by preparing materials, progress reports, and data analyses for active projects and initiatives.
  • Assist and advise the VP in implementing operational policies and procedures, ensuring operational efficiency is maintained throughout the program.
  • Advise the VP on operational issues related to program activities and recommend strategic solutions.
  • Review projects in progress and/or at completion to evaluate effectiveness, ensure compliance, accuracy, and timeliness.
  • Participate in internal operational and leadership meetings for future planning, data review, and issue discussion.
  • Contribute to special projects and perform other duties as needed.
