The Associate Behavioral Health Care Manager (BHCM) is an entry-level role within Colla Health's care model. These individuals act as trusted advocates, providing targeted behavioral health support that integrates with a patient's physical healthcare. They serve as a crucial link between the entire care team, including primary care providers and psychiatric consultants, ensuring a unified and compassionate care plan. The Associate BHCM will deliver care to cancer patients using the Collaborative Care Model, receiving guidance and support from senior clinicians to enhance their skills and clinical expertise. This is a fully remote position, supporting one or more partner practices, with a Monday-Friday work schedule.
