About The Position

The Assisted Living Nurse Manager is responsible for overseeing the clinical and operational coordination of the Assisted Living program while ensuring residents receive high-quality, person-centered care. This position serves as the primary clinical liaison for prospective and current residents and families, managing the admissions and discharge processes, conducting assessments, coordinating service plans, and supporting regulatory compliance. The ideal candidate is a licensed nurse who is compassionate, organized, customer-service focused, and experienced in working with older adults in an assisted living or long-term care setting.

Requirements

  • Current, active RN or LPN license in the state of Ohio.
  • Minimum of one (1) year of nursing experience required.
  • Strong assessment, communication, customer service, and organizational skills.
  • Ability to work collaboratively with residents, families, physicians, and interdisciplinary teams.
  • Proficient with electronic medical record systems (PCC) and Microsoft Office applications.

Nice To Haves

  • Previous experience in assisted living, long-term care, skilled nursing, or senior services preferred.
  • Knowledge of assisted living regulations and resident assessment processes preferred.

Responsibilities

  • Coordinate and manage all resident admissions, transfers, and discharges.
  • Conduct pre-admission assessments and determine appropriateness for admission.
  • Complete home assessments as needed prior to admission.
  • Meet with prospective residents and families, conduct community tours, and assist with the move-in process.
  • Develop, update, and monitor resident service plans in accordance with state regulations and resident needs.
  • Communicate effectively with physicians, families, residents, and interdisciplinary team members.
  • Monitor residents for changes in condition and ensure appropriate interventions are implemented.
  • Maintain accurate resident records and required documentation.
  • Ensure compliance with state regulations, company policies, and resident rights standards.
  • Participate in care conferences and resident-centered planning meetings.
  • Assist with staff education and clinical guidance as needed.
  • Promote resident satisfaction and a positive community experience.
