About The Position

The Assisted Living Coordinator is responsible for planning, developing, organizing, implementing, and evaluating all assisted living programs. This role involves overseeing the coordination of outside nursing services, maintaining job descriptions for staff, and developing the assisted living budget in conjunction with the Executive Director. The coordinator ensures departments function within budgetary limitations and oversees the safety program, including OSHA compliance and case management. They also coordinate with the Wellness Director and discharge planners for safe resident transitions, maintain contact with residents and families during hospitalizations, and act as a resident advocate. A key responsibility is developing and evaluating individual resident care plans with resident participation. The role also manages pharmaceutical services, conducts medication audits, and participates in the admissions and assessment process. Regular individual resident meetings, ongoing needs assessments, and reassessments are crucial. The coordinator manages the Assisted Living staff, determines staffing needs, and delegates authority to supervisors. Ensuring professional personnel have valid licenses, reviewing complaints/grievances, and administering disciplinary action fairly are also part of the role. They conduct staff meetings, develop in-service training, and implement orientation programs for new employees. Encouraging teamwork and leadership training, attending continuing education, and ensuring staff participation in required training are essential. Adherence to HIPAA and monitoring daily living care to ensure residents are treated with kindness, dignity, and respect are paramount. Finally, the coordinator reviews and investigates resident complaints and grievances, reporting resolutions to the Executive Director and Wellness Director.

Requirements

  • Valid and current professional licenses as required by the state.
  • Participation in continuing education programs required by the state for licensed and/or certified staff.

Responsibilities

  • Plan, develop, organize, implement, evaluate, and direct all assisted living programs.

  • Develop, assess, evaluate, implement, etc., individual resident care plans with resident participation.
  • Manage pharmaceutical services communications and participate in obtaining refills or new prescriptions to ensure accurate medication administration.
  • Conduct periodic cart audits, manage the adherence to written policies, and procedures for the administration, storage, disposal and control of medications.
  • Participate in the admissions and assessment process prior to assisted living move-in.
  • Oversee medication management and timely paperwork filing into existing resident medical records to maintain accuracy.
  • Meet with each resident individually on a regular basis; Assess ongoing needs of residents and perform reassessments when warranted and scheduled.
  • Manage the staff in the Assisted Living community.
  • Determine the staffing needs for assisted living necessary to ensure safety and care for the residents.
  • Delegate to supervisors the administrative authority, responsibility and accountability necessary to perform their assigned duties.
  • Ensure that professional personnel have valid and current licenses as required by this state.
  • Review of complaints/grievances made or filed by department personnel.
  • Ensure disciplinary action is administered fairly and without regard to race, color, creed, national origin, age, sex, handicap, or marital status.
  • Conduct regularly scheduled staff meetings.
  • Develop and participate in the planning, conducting, and scheduling of timely in-service training classes.
  • Develop, implement and maintain an effective orientation program to orient new employees to the facility, its policies and procedures, job position and duties.
  • Providing opportunities for and encouraging teamwork and leadership training is essential for all licensed personnel.
  • Attend and participate in continuing educational programs designed to keep abreast of changes in the nursing profession as well to maintain licensure status. Attend any training deemed mandatory by the state.
  • Require all licensed staff who are licensed and/or certified to participate in continuing education programs required by the state. Keep accurate records of participation.
  • Maintain adherence to HIPPA.
  • Monitor activities of daily living care and the facility to assure all residents are treated fairly and with kindness, dignity, and respect and ensure resident respond in kind to staff.
  • Review/investigate complaints and grievances made by residents and present a written/oral report to the Executive Director and Wellness Director indicating resolutions.
