Assisted Living Care Coordinator

Our mission is to enhance the quality of life for seniors by providing a safe, caring, and supportive environment. As an Assisted Living Care Coordinator, you’ll provide care and support in a cluster care setting—not one-on-one private duty. You will work closely with our interdisciplinary team to assist residents with Activities of Daily Living (ADLs) – bathing, dressing, grooming, toileting, light housekeeping and meal support, resident escorting within the facility, and social engagement and companionship.
Requirements
- Active NYS Home Health Aide Certificate (Required)
- Compassionate, dependable, and team-oriented
Nice To Haves
- Experience in Assisted Living is a plus, but not required
Responsibilities
- Provide care and support in a cluster care setting
- Assist residents with Activities of Daily Living (ADLs) – bathing, dressing, grooming, toileting
- Provide light housekeeping and meal support
- Escort residents within the facility
- Provide social engagement and companionship
