About The Position

Genesis has built a full-spectrum MSK platform including surgery, therapy, and imaging, operating across 16 clinics in Chicagoland, serving Medicaid patients at scale. The company is now focused on integrating rehabilitation into this platform to be tightly integrated, protocol-driven, financially sustainable, and deeply accessible. The Assistant Vice President (AVP) of PT & OT is a critical operational leadership role responsible for owning all aspects of Physical Therapy and Occupational Therapy, including strategy, operations, finance, talent, clinical quality, and growth across a rapidly expanding multi-site therapy operation. The AVP will report directly to the COO, manage Clinic Managers and therapy office staff, and bridge clinical excellence with operational performance. This role is not purely administrative, requiring the AVP to maintain 10–25% direct clinical productivity to stay connected to clinical practice and model expectations.

Requirements

  • Licensed PT or OT in Illinois (or getting there)
  • 7+ years of progressive leadership experience in rehabilitation services
  • Real multi-site operational chops
  • Understand outpatient therapy economics, billing, and reimbursement well enough to own a P&L and defend it
  • An operator who came up through the clinic
  • Ability to read a dashboard and read a patient
  • A builder who sees the gap and feels the urgency to close it
  • Runs toward data, doesn't manage by instinct alone
  • Builds dashboards, tracks KPIs, and holds teams accountable to metrics that actually connect to patient outcomes and financial performance
  • Develops people (Clinic Managers)
  • Mission-aligned (serving Medicaid or underinsured patients at the same standard as everyone else)
  • Bachelor’s degree required

Nice To Haves

  • Orthopedic or MSK experience
  • Experience in value-based care models (bundles, capitation, episodic payments)
  • Familiarity with Illinois Medicaid and commercial payer dynamics
  • Advanced degree (DPT, OTD, MBA, MHA, or related) strongly preferred
  • Demonstrated success scaling therapy operations

Responsibilities

  • Own day-to-day operations across all PT/OT clinics: staffing, scheduling, patient throughput, and workflow standardization that actually scales
  • Integrate therapy seamlessly with orthopedic clinics — reducing referral leakage and keeping patients inside the Genesis care pathway
  • Build coverage models that work: in-person, hybrid, and tele-rehabilitation where the evidence and the patient population support it
  • Expand the PT/OT footprint across Chicago and surrounding markets — identifying new locations based on referral patterns and payer mix
  • Build systems that ruthlessly optimize speed-to-first-visit and patient retention
  • Extend hours, add sites, and remove every friction point that stands between a patient and their first appointment
  • Partner with Genesis surgeons, PM&R physicians, and APPs to build protocol-driven rehabilitation pathways by diagnosis and procedure
  • Embed PT/OT early — prehabilitation, conservative management, post-operative care — so therapy is part of the patient journey from the start, not an afterthought
  • Align utilization with evidence-based guidelines and value-based care objectives
  • Own the P&L for PT/OT service lines: revenue, expenses, contribution margin
  • Optimize therapist productivity and reimbursement performance
  • Build prior authorization discipline — timely submissions, minimal denials, tight alignment between approved visits and care plans
  • Drive conservative management strategies that reduce total cost of care — for the patient and the system
  • Build and continuously refine operational dashboards that give leadership real-time visibility into what's working and what isn't
  • Define and own KPIs across the therapy organization: visits per case, units per visit, therapist productivity, days to first visit, cancellation rates, referral conversion, authorization turnaround, functional outcomes, patient satisfaction
  • Create a culture of data transparency where every Clinic Manager knows their numbers and owns their performance
  • Recruit, develop, and retain high-performing PT and OT clinicians and leaders across all markets
  • Build and maintain academic partnerships with PT/OT programs to keep the hiring pipeline full
  • Coach Clinic Managers to operational, financial, and clinical success
  • Develop scalable staffing models — effective utilization of PTAs, COTAs, and support staff — that grow with the platform
  • Run 90-day OKRs for yourself and your direct reports, aligned to the broader organizational priorities
  • Drive disciplined execution against growth, operational, and financial targets — quarter after quarter
  • Maintain 10–25% direct clinical productivity
