Work with the CN Guidance FACT Team to deliver services to those individuals who are diagnosed with severe mental illness and whose needs have not been met by more traditional service delivery approaches. In absence of Team Leader, direct and supervise staff activities, lead team organizational and service planning meetings and provide direction to staff regarding individual cases. Perform routine team duties, and have lead responsibility for integrating dual-recovery treatment, family support or vocational/employment training with the tasks of other team members. Responsible for providing Assessment and Service Planning, Quality Assurance and Improvement Processes, Utilization Review, incident reporting and coordination between ACT and other systems. Engage in performing assessments, case management, handling crisis intervention, and discharge planning. Mains case records and completing documentation on time and in a matter in compliance with federal, state and agency regulations. Provides direct, Telehealth, face-to-face and/or collateral services to recipients either in the FACT Office or in the community in places and situations where problems arise. Must be available (along with other members of the FACT Team) on an as needed basis 24 hours a day, 7 days a week, including responsibility for periodic pager coverage. Assists recipients in developing appropriate expectations from treatment. Strengthens the ability of the recipient to understand and utilize therapy and find resources after treatment has ended through proper discharge planning and termination process. Exchanges information with other agencies that benefits the recipient, or ensures compliance with regulations and coordination of services. Utilizes and understand the value of supervision, which increases practice skills, promotes better services for the recipient and preserves accountability to the Agency. Support the use of data in clinical supervision to inform decision-making Identify and implement quality improvement trainings and activities as part of professional development goals Ability to cultivate a culture of inclusion for all employees that respects their individual strengths, views and experiences.
