Requirements
- Demonstrate knowledge of social work theory and techniques and understanding of emotional and physical problems of the elderly and family members.
- Possess exceptional skills in organizing and keeping track of details and documentation.
- Basic computer and data entry skills.
- Level 1 AZ Fingerprint Clearance Card
Nice To Haves
- Knowledge of psychosocial theory and medical terminology desired.
Responsibilities
- Provide social services & emotional support to residents & families
- Assist with adjustments, ongoing care & discharge planning
- Collaborate with healthcare staff & community resources
