This position provides ancillary health services and care for residents in the community, including participating in the development of and implementing the services plan for each resident and functioning as a role model and coach for nursing assistants and medication aides. The role requires a Licensed Practical Nurse (LPN or LVN) to provide general care and treatment, conduct nursing evaluations/assessments, complete service plans, administer medications and treatments, and serve as a liaison for residents with physicians and outside nursing staff. Additionally, the position involves completing shift reports, providing shift supervision and coaching to team members, and potentially completing team member evaluations. The role also includes customer service aspects such as being a point of contact for resident healthcare, communicating changes in resident condition, meeting with families, and promoting services to external providers.
