About The Position

Supports the daily operations of the Empowered Voices Program while maintaining a direct caseload of participants. This role integrates program leadership, case management, staff coordination, and compliance oversight to ensure high-quality, person-centered services in accordance with CARF, Community Mental Health, state, and federal standards.

Requirements

  • Bachelor's degree in human services, social work, psychology, or related field, master's degree preferred.
  • Two years of relevant experience; case management experience preferred.
  • Ability to finish CPR and First Aid certification within 90 days of hiring.
  • Valid Michigan driver's license and insurable record; able to secure a valid Michigan Chauffer's License.
  • High proficiency with a computer including use of Microsoft Office.
  • Strong written and verbal communication skills.
  • Ability to work with diverse populations.
  • Ability to display reasoning; decision-making skills, and empathy.
  • Successful criminal history background check and drug screen.

Responsibilities

  • Assist in oversight of daily program operations and service delivery.
  • Support implementation of agency policies, procedures, and quality standards
  • Participate in intake, referral coordination, and service planning.
  • Maintain referral, enrollment, and discharge data.
  • Prepare required reports and documentation.
  • Support audit preparation and corrective actions.
  • Maintain confidentiality and HIPAA standards.
  • Support safety and risk management activities.
  • Provide daily guidance to assigned staff and other program staff, as requested.
  • Assist with staff scheduling and coverage.
  • Participate in onboarding and training.
  • Observe performance and provide coaching.
  • Complete evaluations and corrective actions of assigned staff.
  • Assist in recruitment efforts, including reviewing applications and participating in interviews.
  • Maintain an assigned caseload of participants.
  • Author Individual Plans of Service (IPOS), addendums, and progress reviews.
  • Coordinate and attend Person-Centered Planning meetings.
  • Implement and monitor person-centered IPOS.
  • Complete intake (including Discovery) and orientation of participants to program.
  • Communicate with referral sources, guardians, and families concerning participant's progress and/or problem development.
  • Coach participants on job readiness, workplace behaviors, and life skills.
  • Support employment exploration and barrier reduction.
  • Encourage participant engagement and self-advocacy
  • Provide follow-along and community-based supports.
  • Transport participants as needed.
  • Maintain documentation in compliance with CARF and CMH standards.
  • Report critical incidents promptly.
  • Participate in committees and trainings.
  • Support community outreach.
  • Assist with entering encounter notes into EMR
  • All other duties as assigned.
