About The Position

Rediscovery is a therapeutic-based group home with a focus on independent living skill building for males between ages 16-21. Youth at Rediscovery are able to safely access the community independently and are able to utilize learned coping mechanisms with the support of our comprehensive team including clinical and direct care staff. In this position, you will be able to convey your passion for helping others with a dynamic team by bringing an energetic and positive approach while working with our students!

Requirements

• High school diploma or equivalent.
• Experience working in a residential, human services, or behavioral health setting.
• Current MAP certification preferred; willingness and ability to obtain/maintain MAP certification required.
• Strong attention to detail and ability to accurately maintain medication and medical documentation.
• Ability to follow established policies, procedures, and medication administration protocols.
• Strong organizational and time-management skills.
• Ability to communicate professionally with pharmacies, medical providers, staff, youth, and program leadership.
• Demonstrated ability to maintain confidentiality and appropriately handle sensitive medical information.
• Basic computer proficiency and ability to learn electronic health record/documentation systems.
• Ability to identify and communicate medication-related concerns or discrepancies to the appropriate supervisor.
• Ability to work independently while also functioning effectively as part of a team.
• Reliable attendance and consistent follow-through are essential.
• Background and driving record checks will be performed.

Responsibilities

• Work closely with and under the direction of Senior MAP personnel to support medication administration and compliance at the program.
• Conduct weekly MAP audits to ensure documentation and practices are accurate and compliant with program requirements.
• Monitor medication supplies and communicate with the pharmacy.
• Support the maintenance of accurate medication records, including updating medication changes and creating or revising medication sheets as needed.
• Coordinate and schedule medical appointments for youth, ensuring appointments are tracked and communicated appropriately.
• Maintain and update youth height and weight records, and ensure measurements are accurately documented.
• Provide physical interventions and verbal de-escalations to provide the safest therapeutic atmosphere for our students!
