Salinas Valley Post Acute is seeking a motivated and experienced Assistant Director of Rehabilitation (Physical Therapist) to join our dynamic rehabilitation team. Requirements

  • Graduate of an accredited Physical Therapy program.
  • Current and unrestricted Physical Therapist (PT) license in the State of California.
  • Strong clinical assessment, documentation, and care planning skills.
  • Excellent communication, organization, and interpersonal abilities.
  • Ability to work effectively within an interdisciplinary team environment.

Nice To Haves

  • Previous experience in a skilled nursing facility, post-acute care, rehabilitation center, or long-term care setting preferred.
  • Leadership or supervisory experience preferred.
  • Knowledge of Medicare, Medicaid, and rehabilitation documentation requirements preferred.

Responsibilities

  • Evaluate residents to determine physical therapy needs, functional limitations, and rehabilitation potential in accordance with physician orders and facility policies.
  • Develop, implement, and update individualized treatment plans focused on improving mobility, strength, balance, endurance, and functional independence.
  • Assist the nursing department with training and oversight of Restorative Nursing Aides.
  • Supervise Physical Therapist Assistants (PTAs), rehabilitation aides, and students in accordance with state practice regulations.
  • Communicate regularly with physicians, nursing staff, residents, families, and interdisciplinary team members regarding treatment progress, goals, and discharge planning.
  • Participate in Resident Care Conferences, Utilization Review meetings, Rehabilitation meetings, and other interdisciplinary care planning activities.
  • Provide in-service education and training programs for facility staff as needed.
  • Complete timely, accurate, and compliant clinical documentation in accordance with facility standards, regulatory requirements, and payer guidelines.
  • Maintain a safe treatment environment by adhering to infection control procedures, safety standards, and equipment maintenance protocols.
  • Educate residents, families, and caregivers regarding treatment goals, home exercise programs, safety techniques, fall prevention strategies, and maintenance programs.
  • Recommend and facilitate the procurement of durable medical equipment (DME) to support resident mobility and independence, including walkers, canes, splints, grab bars, and adaptive devices.
  • Maintain strict confidentiality of resident and company information.
  • Report equipment issues promptly to ensure proper maintenance and safe operation.
  • Support quality assurance initiatives, performance improvement programs, and rehabilitation department development efforts.
  • Foster positive and professional relationships with residents, families, staff, physicians, and community partners.
  • Provide supervision and clinical guidance to Physical Therapist Assistants (PTAs), rehabilitation aides, and therapy students.
  • Support rehabilitation department operations and assist with staff development, scheduling, and program implementation as directed.
