VNA is looking for a Assistant Director of Patient Care Services. Job Summary: The Assistant DPCS leads home health clinicals and supports staff to deliver safe, compliance, patient-centered care while promoting accountability and consistent clinical and operation performance. This role identifies learning needs, develops and coaches staff, supports regulatory compliance, addresses performance concerns, and guides teams through change. In collaboration with the Director of Patient Care Services, Quality Assurance, program leaders, and the interdisciplinary care team, the Assistant DPCS oversees care coordination, utilization, OASIS and coding, staffing, productivity, financial stewardship, and safe transitions of care to achieve quality outcomes while respecting patient choice.

Requirements

  • Current, unrestricted California Registered Nurse license and current CPR certification required.
  • A minimum of three of the past five years of nursing experience in home health, including at least one year in a supervisory or clinical leadership role, is required for DPCS-designee consideration.
  • Strong educator and communicator with the ability to assess learning needs, explain complex clinical and regulatory requirements clearly, adapt teaching to varied experience levels, provide constructive feedback, and verify that learning is applied in practice.
  • Working knowledge of California Title 22, Medicare home health requirements, Medi-Cal requirements, OASIS, utilization management, payer requirements, and agency policies and procedures.

Responsibilities

  • Seek input from program managers and department leaders, communicate decisions clearly, and build productive working relationships that support shared clinical and operational goals.
  • Assess learning needs; deliver and coordinate orientation, onboarding, clinical education, and ongoing training; validate competency; and ensure assigned staff meet all compliance and practice requirements.
  • Monitor staffing assignments, schedules, caseloads, productivity, and resource utilization; anticipate barriers; and make timely decisions that support quality outcomes and responsible episode management.
  • Maintain timely, accurate communication and documentation among patients, families, clinicians, physicians, referral sources, and other members of the care team.
  • Lead regularly scheduled caseload reviews, team meetings, and case conferences; use clinical scenarios as teaching opportunities; and ensure follow-up on complex, high-risk, or problem-prone cases.
