About The Position

The Assistant Director of Nursing (ADON) is responsible for ensuring that patient care is coordinated and managed appropriately. The ADON oversees the delivery of care and services and supervises clinical personnel. This role involves developing and initiating plans of care, conducting assessments, and ensuring that preventative and rehabilitative nursing procedures are implemented as ordered by physicians. The ADON also plays a key role in case management, including referral review, patient information assessment, and assignment of home care personnel. They collaborate with physicians and clinicians to establish therapeutic goals and ensure quality care delivery. Additionally, the ADON participates in record reviews, assists with screening and interviewing new personnel, and supports the Director of Nursing in educational programs. They are expected to comply with professional standards, participate in quality improvement initiatives, and engage in public relations and community activities to promote the organization's role in healthcare.

Requirements

  • Must be a registered nurse, with current licensure to practice in the State.
  • Three years of management experience in a home health or related health care organization.
  • Demonstrated ability to supervise and direct professional administrative personnel.
  • Ability to market and deal tactfully with customers and the community.
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or Organization requirements and is in good working order.
  • Has excellent observation, verbal, and written communication skills.
  • Knowledge of business management, state, and federal governmental regulations.

Responsibilities

  • Develops, and initiates the plan of care and assumes responsibility for the ongoing interdisciplinary assessment and development of the individualized plan of care in partnership with the patient, representative (if any), and caregiver(s).
  • Initiates proper preventative and rehabilitative nursing procedures and provides services that are ordered by the physician as indicated in the plan of care.
  • Ensures a Clinical Manager/Supervisor is available during all operating hours.
  • Assures the development, implementation, and updates of the individualized plan of care, which would entail communication with all physicians involved in the plan of care and integration of orders, including medication orders, from all physicians involved in the plan of care.
  • Receives case referrals and accepts those who the agency is capable of providing services for.
  • Reviews available patient information related to case, including home visits, to determine home health care needs.
  • Assigns appropriate home care personnel to case as needed.
  • Conferences with Attending Physician regarding any questions about an individual's eligibility for services.
  • Reviews and evaluates each case by reviewing the services provided by clinicians, conferences, record review, discusses and verifies impressions, instructs, and guides clinicians to promote more effective performance and delivery of quality home care services.
  • Is available at all times during operating hours to assist clinicians as appropriate.
  • Reviews patient's medical diagnosis, procedures, medications, and clinical course.
  • Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plan of care.
  • Attends and runs and documents case conference meetings with organization personnel to facilitate coordination of care.
  • Participates in quarterly record reviews and communicates findings and recommendations to Director of Nursing and appropriate organization personnel.
  • Assists in the screening and interviewing process of new organization personnel.
  • Makes recommendations for employment of individuals.
  • Assists in the orientation of new organization personnel.
  • Assists Director of Nursing in the planning, implementation, and evaluation of in-service and continuing education programs.
  • Complies with accepted professional standards and principles.
  • Participates in public relation and community activities that promote the Organization's role as an effective member of the health care delivery system.
  • Participates actively in quality assessment performance improvement program.
  • Performs other duties and activities as delegated by the Director of Nursing and/or Executive Director.
