About The Position

The Transitional Housing program at Veterans Place provides up to two years of residential support for Veterans who are homeless or at risk of homelessness. The Assistant Case Manager is the on-site direct care worker, working to enhance the quality of life for our veterans while supporting the mission of the organization. The position requires an individual who is flexible enough to assume a variety of roles as dictated by the changing needs of our Veterans. It is crucial that an individual be able to relate well to others and handle administrative responsibilities. The Assistant Case Manager is more than just a friendly presence; she/he is the crucial link in creating an environment in which our Veterans heal, grow and prepare to re-enter society as productive, self-sufficient individuals.

Requirements

High School Diploma or equivalent
Must have a valid driver's license
Passing a pre-employment drug and alcohol screening is required and a background check will be conducted
Ability to focus on Recovery Support program details, follow through with timelines, and attend to multiple tasks and interactions.
Ability to retain and recall information regarding clients, policies, procedures, and schedules.
Capable of identifying issues, evaluating solutions, and making appropriate decisions independently or collaboratively.
Ability to comprehend written and verbal information, express thoughts clearly, and communicate effectively with diverse populations (e.g., staff, veterans)
Capacity to manage time effectively, maintain accurate records, coordinate meetings.
Adequate visual acuity (with or without corrective lenses) to read documents, use a computer, and observe participants in meetings or events.
Adequate auditory ability (with or without assistive devices) to understand conversations in person, over the phone, or in virtual meetings.
Clear verbal expression to articulate expectations, deliver feedback, and conduct presentations
Ability to use hands for basic tasks, including writing, typing, and handling documents.
Must be able to lift, carry, and move materials weighing up to 50 pounds regularly using proper lifting techniques.
Required to stand or walk for 6 to 8 hours daily on concrete surfaces.
Ability to sit and use computer for extended periods (4+ hours) with breaks.
Job functions include working outdoors in varying weather conditions including heat, cold, and rain.

Experience working with the public and/or in a customer service capacity strongly preferred.

Responsibilities

Acts as the primary van driver when on duty and provides transportation services to veterans according to program, appointment, and community needs.
Assesses needs and interests of Veterans Place residents by interacting with them on a regular basis.
Initiates programs, activities and community development that address the social, recreational, educational and development aspects of the residents' lives and promote positive relationships, sobriety and supportive community engagement.
Assists with orientation and welcoming of new residents.
Works with staff members to promote responsible living and a culture of sobriety at Veterans Place and is a positive role model in both professional and personal behavior.
Assists in collection and delivery of drug screen samples to the Veterans Administration for processing.
Monitors physical facilities, conducts building rounds as required and identifies maintenance issues, report issues in Building Engines in a timely fashion.
Maintains accurate and timely documentation of all Veteran-related interactions and shift activities, including DAUs, medication counts, groups, inspections, and other required program documentation.
Completes shift reports and incident reports as required. Conducts random room searches, documents findings, and confiscates prohibited/contraband items in accordance with program policy, immediately reporting incidents and confiscated items to Director of Reintegration Services, Transitional Housing Manager and the ACM Coordinator.
Assists with room turnovers and conducts room and refrigerator inspections to ensure cleanliness, safety, and compliance with program standards.
Maintain case records in accordance with program guidelines and professional requirements.
Follow legal and ethical standards of service delivery, privacy, and confidentiality, complying with all applicable regulations.
Ensure safety and security of residents, staff and facility.
Communicates important and arising issues to Milieu Coordinator and, where needed, participates in development of an appropriate course of action.
Communicate policies and the rationale behind them to residents.
Participate in administrative, staff, clinical, and in-service activities and training as required.
Collaborates with Case Managers to provide support and reinforce program expectations, ensuring Veterans remain compliant with program guidelines.
Communicates concerns, behavioral issues, and compliance-related matters to Case Managers and supervisory staff in a timely manner.
