About The Position
Liberty Healthcare is seeking a full-time Assessor to support and care for people with developmental disabilities and autism. This field-based role allows for primary remote work from home, with required travel to client locations throughout the region. The position involves helping individuals with disabilities receive necessary supports and services to achieve personal goals and optimize health and independence.
Responsibilities
- Leading in person and virtual meetings with people who have intellectual and developmental disabilities and/or autism, as well as their family members and caregivers
- Gathering and interpreting pertinent information from individuals and their families and caregivers
- Using standardized assessment tools to help determine needs for services and supports
- Completing structured reports within established deadlines

- Annual compensation of $48,000 (paid $23.07 per hour)
- $500 new hire sign on bonus ($250 paid at 90 days and $250 paid at 180 days)
