About The Position

Liberty Healthcare helps people who have disabilities receive supports and services so they may achieve their personal goals while optimizing their health and independence. Liberty Healthcare is now hiring for a full-time Assessor position. This is a field-based role which will allow you to primarily work remotely from home and also require you to travel to client locations throughout the region.

• Field-based role requiring travel to client locations
• Ability to work remotely from home

Responsibilities

• Leading in person and virtual meetings with people who have intellectual and developmental disabilities and/or autism, as well as their family members and caregivers
• Gathering and interpreting pertinent information from individuals and their families and caregivers
• Using standardized assessment tools to help determine needs for services and supports
• Completing structured reports within established deadlines
