Assessor Coordinator Degree
About The Position
This position reports to the Manager or his/her designate and is responsible for the assessment and coordination of Home Care Client services and case management. The assessment process forms the basis for all activities performed by Home Care, as well as referrals to other appropriate agencies and long-term placement. The position responsibilities and functions are fulfilled with an underlying philosophy of engaging the client and family members, encouraging client empowerment and self-efficacy. Client empowerment is achieved with self-care models, and utilizing Client Centered Care Planning with client goals and outcome. Confidentiality and HIPA guidelines are maintained at all times. Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement.
Requirements
- Degree in a health or social science related field
- Undergraduate degree in Health or Social Sciences
- Demonstrate Leadership and organizational skills
- Advanced - Computer skills
- Intermediate - Public relations skills
- Written Communication Skills
- Critical thinking skills
- Verbal communication skills
- Knowledge of assessment processes and terminology
- Ability to work well with different types of personalities in various settings
- Knowledge of Assessment and Development care plans
- A valid driver's license and a vehicle is required to carry out duties
- Valid Class 5 driver's license
- Registered/eligible for registration with the appropriate licensing body
- Access to a reliable vehicle
- Upload all relevant documents (ex: certificates, diplomas, proof of enrollment, licenses and/or memberships) relating to the qualifications of the position
- Bring a valid Criminal Record Check (CRC) to the interview; it must be dated within six (6) months and include a vulnerable sector search
