About The Position

Under the direction of the manager and as part of the interdisciplinary team, the Assessor Coordinator – Degree is responsible for the full scope of case management services. Case management is patient-centered, strength based, flexible, cost-conscious and quality driven. One of the key functions of case management is to advocate for and coordinate resources based on assessed needs. The Assessor Coordinator – Degree works within Primary Health Care and is one of the entry points for patients needing access and navigation support to community, home care, long-term care and acute care services. The core functions of case management practice include referral intake, engagement, care planning, monitoring, evaluation, and termination/discharge. The Assessor Coordinator – Degree liaises with other services within the network area and throughout the province, in an endeavor to provide the right service at the right time and in the right place.

Requirements

  • Degree in a health or social science related field.
  • Time management skills
  • Communication skills including ability to provide educational presentations
  • Knowledge of community resources
  • Responsible for accurate observation/documentation of patient related info.
  • Demonstrates effective verbal and written communication skills.
  • Knowledge of social determinants of health affecting vulnerable populations
  • Demonstrates empathy, trust and respect in the provision of care
  • Ability to facilitate applications/participate in integrative case management
  • Knowledge of crisis intervention strategies
  • Knowledge in client case management and process facilitation
  • Advocacy, negotiation, crisis management and conflict resolution
  • Knowledge of Policies, Procedures, Regulations and Legislation pertaining to area of practice
  • Access to a reliable vehicle
  • Registered/eligible for registration with the appropriate licensing body
  • Valid Class 5 driver's license

Responsibilities

  • Responsible for the full scope of case management services.
