The VPH Admissions/Reception department is seeking an Assessment Specialist to perform clinical crisis assessments, telephone triage in the psych access center, referral coordination, and disposition planning for psychiatric patients of all ages. The role requires accurate documentation in the patient record and knowledge to refer patients to the appropriate level of care within the behavioral health continuum. The available shift is 7:00 PM - 7:00 AM.

Requirements

Master's degree
2 years of relevant work experience
LIC-Licensed Clinical Social Worker - Tennessee
LIC-Licensed Professional Counselor-Mental Health Service Provider - Licensure-Others

Responsibilities

Completes Crisis Assessment independently via in person and/or tele psych
Documents accurately in patient chart and has the ability to update the team during shift changes
Facilitates Admissions / assessments with Physician partners
Performs crisis intervention
Telephone Triage Activities
Contributes to the organization by actively participating in process improvement initiatives and shared decision making
