The Pediatric Social Work Assessment team is seeking an Assessment Specialist to perform behavioral health diagnostic and psychosocial assessments using the DSM. This role involves implementing group, family, and individual psychotherapeutic interventions, including crisis intervention, developing treatment and disposition plans, and maintaining contact with families and community agencies. The specialist will also collaborate with the team to achieve patient care management and system improvement goals, and represent the behavioral health enterprise on institutional and community committees.

Requirements

  • Licensed Professional Counselor - Mental Health Serivces Provider Temporary License (LPC - MHSP TEMP) - State of Tennessee
  • LIC-Licensed Master of Social Work - Tennessee
  • 1 year of Relevant Work Experience
  • Master's degree

Responsibilities

  • Provides initial assessment and develops plan of care according to TN Department of Mental Health, The Joint Commission, IPFQR, HBIPS, and CMS Best Practice Guidelines and within the scope of their license by the Tennessee Health Related Boards.
  • Provides targeted evidence based interventions into clinical practice with patients.
  • Facilitates transitions to appropriate levels of care via in person, telephone or telehealth contact.
  • Maintains professional documentation which meets department and VUMC standards as it relates to compliance.
  • Acts as clinical liaison with patient, family, treatment team and community.
  • Maintains confidentiality and privilege in accordance with TN state codes and statutes.
  • Upholds ethical standards in accordance with relevant professional association’s code of ethics.
  • Contributes to the organization by actively participating in process improvement initiatives and shared decision making.
  • Engages in regularly scheduled supervision as required by the Tennessee Health Related Boards.
