About The Position

WellBridge Healthcare Fort Worth, part of Lifepoint Health, is seeking an Assessment Specialist for their PRN Weekend team. This role is vital in facilitating the response to service requests, including initial response, assessment, referral, determining the appropriate level of care, admission, and follow-up. The specialist will conduct initial evaluations, document patient needs, and provide information for medical director level of care determinations. Excellent customer service skills are essential for assisting patients, physicians, visitors, and coworkers. Knowledge of Emergency Detention paperwork, processes, and EMTALA is required. The role also involves accurately maintaining pre-admission and admission paperwork and completing pre-certifications with insurance companies.

Requirements

• Associate’s Degree in Nursing (required) or a Master’s Degree in Social Work/Counseling (required)
• Current clinical, social work or RN license as required by state regulations
• CPR and de-escalation certification required within 30 days of hire and prior to any patient contact

Nice To Haves

• Prior experience in a psychiatric healthcare facility working with adult and geriatric patients preferred
• Must have strong clinical assessment skills

Responsibilities

• Facilitates response to a request for services, including the initial response, the assessment, and referral process, the designation of the appropriate level of care, admission into services, and appropriate follow up activity
• Conducts initial evaluation, initiates the assessment, documents patient symptomology, functioning, and needs and provides information to MD for LOC determination
• Demonstrates excellent customer service skills in assisting patients, physicians, visitors, and coworkers in a prompt and courteous manner
• Possess clear knowledge of Emergency Detention paperwork and processes and EMTALA
• Maintains all preadmission and admission paperwork with accuracy, and completes all pre-certification with insurance companies with insurance companies
