The role includes coordinating with internal departments and external providers, making referrals based on identified needs, and sharing information with the appropriate ongoing support team. After assessments are completed, individuals are typically transitioned to another team or point of contact for ongoing support. Daily work also involves tracking assignments, meeting deadlines, and maintaining clear communication throughout the process. This is an outwardly facing position which would necessitate frequent meetings in the office or in individuals’ homes.
