About The Position

Sycamore Springs is seeking an RN Assessment Specialist/Intake Specialist for their Weekend Midshift. This role is responsible for supporting the department's needs with a focus on clinical, operational, and administrative excellence. The specialist receives inquiry calls, assists callers with scheduling assessments, and triages individuals based on safety risk. The role involves assessing patients for medical and behavioral emergency conditions, performing comprehensive assessments for substance abuse, dual diagnosis, or psychiatric/emotional disorders, and coordinating with the clinical team. Additionally, the specialist completes admission administrative processes and precertification of care. The role also involves administering nursing care, coordinating general and psychiatric nursing care, ensuring patient safety and well-being, following medical orders, initiating treatment planning, and acting as a patient advocate.

Requirements

  • Graduate from an accredited program of professional nursing or relevant state licensure required
  • Current unencumbered license to practice by the State Board of Nursing

Nice To Haves

  • Previous experience in a psychiatric health care facility, with direct experience working with chemical dependency, dual diagnosis, psychiatric and geriatric patients preferred.
  • Experience in patient assessments, family motivations, treatment planning and communication with external review organizations or comparable entities

Responsibilities

  • Receives inquiry calls and assists the caller with scheduling a face-to-face, phone, or tele-assessment.
  • Triages individuals based on safety risk.
  • Assesses or ensures assessment by a qualified mental health professional of patients who present for assessment.
  • Screens for medical and behavioral emergency conditions.
  • Performs a comprehensive assessment and quality therapeutic care to patients seeking treatment for substance abuse, dual diagnosis, or psychiatric or emotional disorders; coordinates with the clinical team/on-call physician to ensure these needs are met at the facility or other appropriate community providers.
  • Interprets treatment to patient and family to help reduce fear and other attitudes obstructing acceptance of care and continuation of treatment.
  • Completes the administrative processes of admission and precertification of care with external payers, as necessary.
  • Administer nursing care to ill or injured persons by assisting in caring for patients by giving direct care.
  • Coordinates and delivers quality general and psychiatric nursing care to patients hospitalized for treatment of substance abuse, dual diagnosis, or psychiatric or emotional disorders.
  • Ensures the safety and well-being of each patient.
  • Ensures all medical orders are followed, initiates and follows treatment planning, and acts as a patient advocate at all times.
