About The Position

DOROT is seeking an Assessment Social Worker to join their Constituent & Community Services (CCS) Department. The CCS department provides a range of supportive services that assist older adults in aging in place and connecting to programs and resources that help them thrive and build vital social connections. The Assessment Social Worker will conduct comprehensive home assessments of new and ongoing older adult participants, identify supportive services, and introduce older adults to DOROT’s programs. This role involves working independently in the community, collaborating with the social work team, and maintaining client records in DOROT’s online database. The position also includes facilitating support groups, managing a caseload of Friendly Visiting matches, providing information and referral services, and representing DOROT at community events. Travel expenses for home visits are covered by DOROT.

Requirements

  • An LMSW or a Licensed Mental Health Counselor
  • Affinity for working with an older adult population
  • Strong interpersonal, clinical, group facilitation, and presentation skills
  • Strong written and verbal communication skills
  • Strong organizational skills, with an ability to work independently and collaboratively
  • Passion and sensitivity for DOROT’s mission
  • Availability to work approximately 8 Sundays and/or work nights per year for programs or events; this includes up to four events that may not be within your regular scope of work
  • Proficiency with Microsoft Office Suite

Responsibilities

  • Conduct in-home assessments working independently in the community.
  • Review program opportunities that support the older adult’s priorities and ability to meet program criteria.
  • Provide short-term case assistance, information, referral, and advocacy to older adults and their caregivers following the assessment.
  • Collaborate with the social work team and other staff to initiate the process for program participation(s).
  • Collect, track, and maintain case assessment and related client interactions using DOROTs’ online database; ensure accuracy and detail of records.
  • Facilitate or co-facilitate one or more support or psychoeducational multi-session groups each year.
  • Serve as the assigned social worker for a caseload of up to 20 Friendly Visiting matches, conducting quarterly check-ins and ad-hoc support to participating older adults and volunteers.
  • Deliver case assistance to and conduct advocacy for older adults, their caregivers, volunteers, and other constituents, to help meet their needs.
  • Provide Information & referral to those in need of support beyond our scope of work.
  • Stay up to date on relevant issues that relate to aging in the community.
  • Supervise both interns and office volunteers as needed.
  • Represent DOROT at community outreach events, networking opportunities and conferences.
  • Collect, track and maintain case assessment and related client interactions using DOROT online database (Salesforce); this includes routine data entry and timely case note documentation.
  • Information & Referral helpline coverage, special projects and other duties, as assigned.
