About The Position
Heritage Behavioral Health Center is hiring passionate professionals for the role of Assessment Clinician within their Community Transition program. This is a mission-driven Certified Community Behavioral Health Clinic located in Decatur, Illinois, dedicated to improving mental health and substance use care. The program focuses on assisting individuals with severe and persistent mental illness/dual disorders to transition from nursing facilities to community settings, with services primarily provided within partnered nursing facilities. Heritage Behavioral Health Center emphasizes a collaborative work environment and believes in supporting its staff to ensure they can effectively care for clients.
Requirements
- Master’s degree in social work, human services, or related field required
- Minimum of 21 years of age
- A valid driver’s license, reliable means of transportation, and proof of current automobile insurance required

Responsibilities
- Completion of assessments and making appropriate recommendations for persons served residing in the nursing facility
- Collaboration with nursing facility staff to review records and gather information
- Participation as a member of the multi-disciplinary clinical team
