About The Position

Terros Health is seeking an Assessment Clinician for its Javelina location in Mesa, AZ. This full-time role is within the Behavioral Health/Social Work category and focuses on providing comprehensive biopsychosocial assessments for justice and crisis-referred clients. The clinician will work closely with the Maricopa County Adult Probation Department, Bridging the Gap (BTG) Re-entry and Crisis Aftercare Programs, and other community contacts to determine client diagnoses and treatment needs. The position involves developing Service Plans through a team format to improve health outcomes and may include providing evidence-based therapeutic interventions such as outpatient assessment, diagnosis, and counseling for adults.

Requirements

  • Master’s degree (MA, MC, MSW) in a behavioral health field.
  • Arizona behavioral health license (other than LBSW) or eligible for Arizona license from Arizona Board of Behavioral Health Examiners.
  • Ability to perform first aid or cardiopulmonary resuscitation as necessary and appropriate.
  • Knowledge in use of Electronic Health Record (NextGen a plus).
  • Proficiency using the MS Office suite, particularly Word and Outlook.
  • Must maintain a valid Arizona driver’s license, be 22 years of age with minimum 3 years driving experience, and meet requirements of Terros Health’s driving policy.
  • Must pass background check in accordance with Terros Health and DCS requirements (no probation or parole within the last 10 years).
  • Must have Arizona Fingerprint Clearance card or apply for one within 7 working days of assuming role.
  • Must pass DCS Central Registry check without any disqualifying events.
  • Must pass a TB Test.
  • Experience working with diverse populations.

Responsibilities

  • Reviews referral and supplemental documents prior to assessment to gain insight about the patient.
  • Conducts a comprehensive biopsychosocial and ASAM Criteria® assessment to identify treatment needs and level of care.
  • Requests additional information from the referral source if treatment is not recommended.
  • Coordinates with the referral source after each assessment, communicating treatment recommendations and concerns.
  • Makes referrals to appropriate facilities if ASAM Criteria® assessment indicates a need for stabilization, inpatient, or residential services.
  • Prepares a written assessment summary detailing findings and treatment recommendations.
  • Completes coordination of care activities with other entities to ensure client needs are identified and addressed in the Individualized Service Plan.
  • Coordinates care for the patient after each assessment and connects them to appropriate Terros Health team members.
  • Conducts therapeutic services using evidence-based interventions, which may include individual, couple, family, or group service methods, if designated.
