The Assessment Field Support Nurse (AFSN) is a full-time role responsible for skilled and non-skilled patient assessments, providing oversight and support for patients receiving caregiving, home health, and private duty care services. Reporting to the Director of Nursing (DON), this position acts as a clinical resource for patients, families, caregivers, and nursing staff, ensuring quality care, regulatory compliance, and service continuity. Key responsibilities include conducting initial and ongoing assessments, supporting the admission process, providing clinical education and competency validation, and collaborating with the DON on quality improvement initiatives. This role is field-based and serves as a liaison between patients/families, field staff, and the DON to ensure exceptional clinical care and patient satisfaction. It does not involve employee performance management, disciplinary actions, scheduling, or direct staff supervision, but rather focuses on assessments, education, support, and quality.
