Provide direct patient care and support Assist in developing and implementing treatment plans Monitor patient behavior and document progress Collaborate with clinical staff to ensure quality care facilitate daily team meetings assuming an active role in screening referrals and assessing beneficiaries at intake overseeing the administrative operations of the team providing direct services to ACT service beneficiaries, where a therapeutic relationship is developed between ACT service beneficiaries and the Team Leader On-Call Crisis support
